Intervention Worker – Housing  

 

Position Summary 

The Intervention Worker provides housing support to individuals experiencing or at risk of homelessness. The role focuses on preventing homelessness, promoting housing stability, addressing immediate needs, and connecting individuals to IHCT services, specifically related to mental health and addictions. 

 

Key Responsibilities 

  • Prevent homelessness by intervening in situations involving imminent housing loss and supporting individuals transitioning from institutions such as hospitals, correctional facilities, treatment programs, or other residential settings. 

  • Assist individuals experiencing homelessness in securing and maintaining safe, stable, and appropriate housing through individualized support plans and housing navigation services. 

  • Facilitate access to IHCT services by connecting clients with the organization's medical clinic for primary care appointments, mental health services, addiction counselling, and other health-related supports. 

  • Conduct outreach activities to engage individuals experiencing homelessness and support them in meeting their immediate basic needs, including access to food, clothing, emergency shelter, and other essential resources. 

  • Provide ongoing case management and follow-up to promote long-term residential stability and improve overall well-being. 

  • Develop collaborative relationships with community partners and service providers to strengthen support networks for clients. 

  • Maintain accurate and timely client records, assessments, case notes, and service plans in accordance with organizational policies and confidentiality requirements. 

  • Participate in multidisciplinary team meetings and contribute to coordinated care planning for clients with complex needs. 

  

Requirements: 

 

  • Diploma or degree in social work, social sciences or a related field 

  • Experience working with Indigenous communities  

  • Experience working with individuals experiencing homelessness, mental health challenges, or substance use issues.  

  • Good knowledge of housing resources and community support.  

  • Understanding trauma-informed care, harm reduction, and recovery-oriented practices.  

  • Strong communication, interpersonal, and relationship-building skills.  

  • Ability to work independently and collaboratively within a multidisciplinary team.  

  • Excellent organizational, time management, and reporting skills.  

  • Ability to maintain confidentiality and professional boundaries.  

  

Working conditions:  

  • Salary to be discussed  

  • Fixed-term contract ending in March 2027 

  • This is not a work from home position. The Intervention Worker works at the IHCT office with regular outreach. 

  • Standard hours will be between 8:30 am to 4:30 pm Monday to Friday, with one half-hour paid lunch break unless otherwise agreed by management. Evenings or weekend hours may also occur. 

  • Overtime hours are accumulated as banked time rather than being paid out. 

 

Employment is conditional upon a satisfactory criminal record check.
