Health Navigation

The Health Navigator program provides support and advocacy to members of First Nations, Inuit and Métis communities living in urban areas by:

Facilitating and coordinating access to health services

Responding to cultural and spiritual needs

Connecting with Indigenous and non-Indigenous partners to ensure a culturally safe care pathway
How can health navigators help me?
Navigators can help you with:

Scheduling medical appointments

Referrals to other services
-
Support in hospitals or other health facilities

Support in filling out forms and obtaining the necessary documents (health card, hospital card, Indian status card)

Emotional and cultural support
-
Access to traditional medicines such as sage, sweetgrass, tobacco and cedar

Connecting with Elders or Knowledge Keepers for cultural support

Advocacy for patients' needs and rights
-
A better understanding of the health system

Translation services as needed
